In this paper, a noteworthy compact dual band wireless power transfer (DB-WPT) system is presented and studied. This system is proposed using interdigital capacitor which reduced the size of the whole system and accomplished high efficiencies. An interdigital capacitor is printed in the ground plane of the design and loaded with a chip capacitor to produce dual bands. The operation of the system is verified by fabricating the proposed DB-WPT system, which shows measured coupling efficiencies of 96 % and 98.8 % at 0.53 and 1.1 GHz, respectively at a transfer space of 12 mm. The fabricated structure has dimensions of 13.5 mm × 20 mm. The results show good agreement between the circuit model and the measurements. The idea of this design makes it applicable for compact near field WPT applications.

Background and aims Thoracic air leak syndrome (TALS) is a complication related to chronic pulmonary graft-versus-host disease (pGvHD) that affects approximately 0.83% to 3.08% patients after allogenic hematopoietic stem cell transplant. Such complication is defined as the occurrence of any form of air leak in the thorax, including spontaneous pneumomediastinum or pneumopericardium, subcutaneous emphysema, interstitial emphysema and pneumothorax and has a negative impact on post-transplant survival. The aim of the present study is to describe a single-center experience in the surgical management of recurrent TALS in adolescents and young adults and its outcome. Methods We retrospectively reviewed the clinical notes of patients with previous allogenic hematopoietic stem cell transplant who underwent surgical procedures for recurrent TALS from January 2016 until March 2021. As well we analyzed clinical data, number of episodes of thoracic air leak, surgical procedures and relative outcome. Results In the examined period, four patients, aged 16 to 25 years, underwent surgical procedures for TALS, including thoracostomy tube placement, thoracoscopic pleurodesis and thoracotomy. All the patients had been diagnosed with pGvHD before the onset of TALS, with a mean time lapse of 276 days (range 42 – 513). These patients experienced on average 4.5 air leak episodes (range 3 – 6). All the patients experienced at least two episodes before surgery. One patient underwent emergency tube thoracostomy only, three patients underwent thoracoscopic pleurodesis and two patients underwent thoracotomy. After surgery, patients were free from air leak symptoms for a mean time of 176 days (range 25 – 477). Pulmonary function progressively deteriorated, and all the patients eventually died because of respiratory failure after a mean time of 483 days (range 127 – 1045) after the first episode of air leak. Conclusions Surgery provides temporary relief to symptoms related to TALS. When TALS develops, pulmonary function progressively worsens toward respiratory failure and death.

A model of the evolution of the onshore wind turbine blade mass installed in Denmark is proposed described by a Weibull distribution and the age of the blades is estimated from decommissioning data to t50 %,onshore = 29 years when half of the blade mass of an installation year has been decommissioned. This is considerable longer than the 20 year design life time of onshore turbines, which is often assumed to be an estimate of the End-of-Life of turbine blades. Thus blade waste predictions using the simple assumption may predict that installed blade masses are entering recycling processes about 9 years sooner that what is observed in Denmark. The blade mass for decommissioning in Denmark is estimate to peak at 2000 ton/year and 5000 ton/year in 2028 and 2045 using the Weibull model.

IntroductionLangerhans cell histiocytosis (LCH) is a rare hematological disorder characterized by intense and abnormal proliferation of bone marrow-derived immature myeloid dendritic cells; Langerhans cells (LCs) in the skin, bone, lymph nodes, and other organs.1Although, due to its low occurrence, limited information on the epidemiology of LCH is available, an estimated 5 to 9 cases per million in children < 15 years of age, and 1 case per million in patients > 15 years of age have been reported.2Depending on the different clinical manifestations, LCH had been classified conventionally by the International Histiocyte Society into three different types: Type I - Eosinophilic granuloma (chronic focal LCH), Type II - Hand-Schüller-Christian disease (chronic disseminated LCH) and Type III - Letterer-Siwe disease (acute disseminated LCH).3 However, because clinical findings are widely manifested, the disease has also been classified into unifocal and multifocal forms; depending on the extent of its spread throughout the body. The unifocal form presents only one destructive lesion in an organ, affecting bones or skin (80% of cases), liver, spleen, brain, and lymph nodes more frequently3 The disease manifests itself in a variety of ways, from isolated disease that resolves on its own to life-threatening multisystem disease, with a 20% mortality rate.4The involvement of head and neck region is quite common in LCH, particularly the bones of skull and jaws. The oral soft tissue lesions are also common and are mostly seen on the hard palate and gingiva. Since the symptoms of LCH may first appear in the oral cavity before occurring elsewhere in the body, a thorough knowledge of the clinical presentation of the disease is important.1Awareness among clinicians regarding the clinical, histological, and immune-histopathological features of the disease will help in early diagnosis and treatment for better prognosis. Thus, this report aims to present a case of Langerhans cell histiocytosis with the oral and dermal manifestations seen in a pediatric patient.

Investigating the conveying and mixing characteristics of high-viscosity fluids in an inclined flow channel under the excitation of acoustic frequency vibrations using computational fluid dynamics (CFD) methods. As the intensity of the acoustic vibration excitation increases, the free surface of the liquid transitions from generating Faraday waves to generating disordered jets. Continued increase in the amplitude leads to the liquid filling most of the vessel space, causing a blockage. However, slowly increasing the amplitude alleviates the blockage phenomenon. When high-viscosity materials are subjected to high-intensity acoustic vibration, the flow field is dominated by shear flow, which is accompanied by efficient stretching and folding. Changing the amplitude or frequency alone can cause blockage, and increasing the frequency of vibration excitation alone will not alleviate the blockage. Instead, increasing the amplitude can generate more mixing-promoting reflux, effectively relieving blockage while maintaining stable conveying capacity.

Purpose: To determine the utility of interferon-gamma-inducible protein 10 (IP-10) for identifying active tuberculosis (TB) and latent TB infection (LTBI) in children in BCG-vaccinated population, establish its diagnostic performance characteristics, and evaluate changes in IP-10 level during anti-TB-chemotherapy. Methods: Concentrations of IP-10 and IFN-γ were measured in QuantiFERON-TB Gold (QFT) supernatants in children with suspected TB or due to a recent TB contact. A total of 225 children were investigated: 33 with active TB, 48 with LTBI, 83 TB contacts, 20 with suspected TB but other final diagnosis, and 41 controls. In 60 children cytokine responses were evaluated on follow-up visit after 2 months of anti-TB-treatment. Results: IP-10 expression was significantly higher in infected children (active TB and LTBI cases) than in uninfected individuals. IP-10 proved effective in identifying TB infection at its optimal cut-off (>1084.5 pg/ml), but was incapable of differentiating between children with active TB and LTBI. Combining IP-10 and IFN-γ increased QFT sensitivity. IP-10 but not IFN-γ decreased significantly during anti-TB-treatment in children with active TB (p = 0.003). Conclusion: IP-10 identifies TB infection and declines during anti-TB-chemotherapy in children. Incorporating IP-10 into new immunodiagnostic assays could improve TB diagnosis and allow treatment monitoring.

This study reviews the augmented Bayesian learning approach for the multi-agent decision mechanism. The decision mechanism includes the probability situation in a dynamic environment. Suggested system considers the problem of data analysis with distributed task allocation in a set of cooperative agents. This paper evaluates the exchange of information and the status of current information within the system and briefly examines the transition from knowledge base to rule base and from rule base to task, analyzing the sending of messages between agents when the information arrives on the system in a dynamic way. Both the current and past received information are taken into consideration together with related information rules are derived in the rule base structure by an augmented Bayesian classifier approach within knowledge base. In this paper, Expected Maximization (EM) algorithm, which is itself a learning method, has been developed and adapted for dynamic agent based data analysis.

Introduction: Postnatal development has the potential to contribute to lung function in adult life, although associations between health-related parameters and lung function remains unclear in childhood. The aim of the study is to evaluate the relationship between physical fitness and anthropometric parameters with lung function of healthy scholar-aged children. Method: A total of 418 children aged 7-8 years old participated in this study. The associations of fitness (handgrip strength, standing broad jump, and 800-m run) and anthropometric (waist circumference (WC) and body mass index (BMI)) parameters with lung function (forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV 1)) were analysed using mixed-linear regression model. All measurements were performed using standard procedures for children. Results: Girls had significantly lower FVC values (p=0.006) and physical fitness (p<0.019) compared to boys. On mixed-linear regression analyses, handgrip strength (p<0.001) and WC (p<0.001) were independently associated with FVC, explaining together 26.6 % of its variance while handgrip strength (p=0.001), WC (p<0.001) and BMI (p<0.001), were independently associated with FEV 1, accounting together for 21.0 % of its variance in 7-year healthy children. Conclusions: Handgrip strength and anthropometric parameters were associated with lung function in healthy children highlighting the influence of torso development in lung function. Our results corroborate the need to promote physical fitness during childhood, especially among girls, in order to enhance their pulmonary function and protect from lung complications in later life.

Objective To test the hypothesis that the fetal fraction in twin pregnancy would reflect birthweight discrepancy. Design Retrospective cohort study. Setting International Peace Maternity and Child Health Hospital in Shanghai, China Population We included 237 twin pregnancies undergoing cfDNA screening for aneuploidy and delivered at International Peace Maternity and Child Health Hospital in Shanghai, China between January 2018 to December 2021. Exclusion criteria including abnormal NIPT results, fetal demise or structural abnormalities. Methods All women with twin pregnancies were offered a scan to determine chorionicity as well as first-trimester nuchal translucency (NT) at 11 +0-13 +6 weeks. Dichorionic was confirmed when ultrasound assessment clearly indicates two placentas. The twin peak sign is used to distinguish chorionicity if only one placenta is visualized. Main Outcome Measures Fetal fraction and birth weight of the new borns were collected. Relationships between fetal fraction and birth weight discordance or sFGR were analysed. Results Fetal fraction was positively correlated with the difference of birth weight (β=0.004, 95%CI: 0.001~0.006). Higher fetal fraction was significantly associated with the increased risk of birthweight discordance of 20% (adjusted OR:1.073, 95%CI: 1.009~1.142) and 25% (adjusted OR:1.092, 95%CI: 1.006~1.185) and sIUGR(adjusted OR:1.130, 95%CI: 1.038~1.231). We obtained the optimum cut-off point of fetal fraction ≥ 11.790, ≥ 14.800 and ≥ 14.800 for birthweight discordance of 20% and 25% and sFGR, respectively. Conclusion This study shown that fetal fraction was positively correlated with the difference of birth weight. Fetal fraction could be used as a biomarker in predating birth weight discordance and sFGR, and help to make individualized clinical monitoring plans for twin pregnancies.

Rotavirus molecular surveillance remains important in the post vaccine era in order to monitor the changes in transmission patterns, identify vaccine induced antigenic changes and discover potentially pathogenic vaccine related strains. The Canadian province of Alberta introduced rotavirus vaccination into its provincial vaccination schedule June 2015. To evaluate the impact of this program on stool rotavirus positivity rate, strain diversity and seasonal trends, we analyzed a prospective cohort of children with acute gastroenteritis recruited between December 2014 and August 2018. We identified dynamic changes in rotavirus positivity and genotype trends during pre- and post- rotavirus vaccine introduction periods. Genotypes G9P[8], G1P[8], G2P[4] and G12P[8] predominated consecutively each season with overall lower rotavirus incidence rates in 2016 and 2017. The demographic and clinical features of rotavirus gastroenteritis were comparable among wild type rotaviruses; however, children with G12P[8] infections were older (P<0.001). Continued efforts to monitor changes in the molecular epidemiology of rotavirus using whole genome sequence characterization is needed to further understand the impact of the selection pressure of vaccination on rotavirus evolution.

Background:The thrombopoietin receptor agonists (TRAs) were recommended for primary immune thrombocytopenia (ITP) during the pandemic of COVID-19. However, the incidence of thrombocytosis and thrombosis was sporadically reported in the chronic immune thrombocytopenia (CITP) patients receiving TRAs during the COVID-19 infection. Objective: With the local prevalence of COVID-19 in Dec 2022 in the Beijing area, we got more powerful evidence about the change in platelet (Plt) counts associated with COVID-19 infection. Methods: A signal center observational cohort study was performed from the beginning of Dec. 2022 to the end of Feb. 2023 to enroll CITP children treated with TRAs alone as the second-line treatment and suffering from the COVID-19 infection in Dec. 2022. The Plt counts before, during, and after COVID-19 infection were collected. Results: In total, 67 (34 males and 33 females) patients with 8.10 (2.15, 15.70) years of age were enrolled. Sixty-three patients who had responded to the TRAs showed a transient increase in Plt counts after the infection of COVID-19. The time of starting to increase was on Day 3(2,7), and to the peak level on Day 14(7,19) of infection with the peak Plt count was 289 (88, 1974)×10 9/L. With at least two months observation period from COVID-19 infection, the Plt counts of 100% (63/63) patients declined to the baseline on Day 25(14, 41). Conclusion: The phenomenon of transient increasing of Plt counts has been shown in the CITP children who have treatment responses to TRAs when suffering from COVID-19 infection.

A 4-year-old child with β-thalassemia major underwent allogeneic hematopoietic stem cell transplantation (HSCT) on August 30, 2022 from a male haploidentical donor. He developed massive pericardial effusion (PE) secondary to a pulmonary infection. Methylprednisolone, tacrolimus discontinued, and pericardiocentesis were all ineffective. Since the interleukin-6 (IL-6) level in the PE was markedly increased (1868.58pg/ml), a total of two doses of tocilizumab was administered intravenously at a dosage of 5mg/kg every 2 weeks. PE remarkably reduced and completely resolved within 8 days. No recurrence of PE was observed in the follow up visits. Our results suggest that tocilizumab can be considered as a treatment of choice in massive PE after HSCT.

RATIONALE: The high sensitivity of the miniature mass spectrometer plays an irreplaceable role in rapid on-site detection. However, its analysis accuracy and stability should be improved due to the influence of sample pretreatment and use environment. The present study investigates the processing effects of EEMD feature enhancement methods on the determination coefficient and relative standard deviation of caffeine mass spectrometry signals. METHODS: This paper employs the EEMD method combined with polynomial curve fitting to enhance the characteristics of seven Caffeine mass spectrum signals with different concentrations and fifteen groups of Caffeine mass spectrum signals with the same concentration, and the wavelet analysis method was used for comparative verification. The determination coefficient and relative standard deviation of the two methods were compared. RESULTS: We found the EEMD method’s capability in adaptively decomposing Caffeine mass spectrum signals is better than wavelet analysis method. The determination coefficient of the EEMD enhanced feature is better than 0.999, and the relative standard deviation is better than 2%, and both are better than wavelet analysis methods. CONCLUSIONS: The feature enhancement processing using the EEMD method has significantly improved the determination coefficient and relative standard deviation of the sample curve, improving the accuracy and stability of the data and providing a new way for miniature mass spectrometer signal processing.

Objectives To investigate the risk of stillbirth in relation to; 1) a previous CD compared to those following a vaginal birth (VB); and 2) vaginal birth after caesarean (VBAC) compared to a repeat CD. Design Population-based cohort study. Setting The Swedish Medical Birth registry Population Women with their first and second singletons between 1982 and 2012. Methods Multivariable logistic regression models were performed to estimate adjusted odds ratios (aOR) and 95% confidence intervals (CI) of the association between CD in the first pregnancy and stillbirth in the second pregnancy and the association between VBAC and stillbirth. Sub-group analyses were performed by types of CD and timing of stillbirth (antepartum and intrapartum). Main outcome measures Stillbirth (antepartum and intrapartum fetal death). Results Of the 1,771,700 singleton births from 885,850 women,117,114 (13.2%) women had a CD in the first pregnancy, and 51,755 had VBAC in the second pregnancy. We found a 37% increased odds of stillbirth (aOR:1.37 [95% CI, 1.23–1.52]) in women with a previous CD compared to VB. The odds of intrapartum stillbirth was higher in previous pre-labour CD group (aOR:2.72 [95% CI, 1.51–4.91]) than the previous in-labour CD group (aOR:1.35 [95% CI, 0.76–2.40,]), although not statistically significant in the latter case. No increased odds was found for intrapartum stillbirth in women who had VBAC (aOR:0.99 [95% CI, 0.48–2.06]) compared to women who had a repeat CD, whereas women with antepartum stillbirth were more likely to have a VBAC (aOR:4.49 [95% CI, 3.55–5.67]). Conclusions This study confirms that a CD is associated with an increased risk of subsequent stillbirth, with a greater risk among pre-labour CD. This association is not solely mediated by increases in intrapartum asphyxia, uterine rupture or attempted VBAC. Further research is needed to understand this association, but these findings might help health care providers to reach optimal decisions regarding mode of birth, particularly when CD is unnecessary.

Epigenetic modifications, like DNA methylation, generate phenotypic diversity in fish and ultimately lead to adaptive evolutionary processes. Euryhaline marine species that migrate between salinity contrasted habitats have received little attention regarding the role of salinity on whole-genome DNA methylation. Investigation of salinity-induced DNA methylation in fish will help to better understand the potential role of this process in salinity acclimation. Using whole genome bisulfite sequencing, we compared DNA methylation patterns in European sea bass (Dicentrarchus labrax) juveniles in seawater and after freshwater transfer. We targeted the gill as a crucial organ involved in plastic responses to environmental changes. To investigate the function of DNA methylation in gills, we performed RNAseq and assessed DNA methylome-transcriptome correlations. We showed a negative correlation between gene expression levels and DNA methylation levels in promoters, first introns and exons. A significant effect of salinity on DNA methylation dynamics with an overall DNA hypomethylation in freshwater-transferred fish compared to seawater controls was demonstrated. This suggests a role of DNA methylation changes in salinity acclimation. Genes involved in key functions as metabolism, ion transport and transepithelial permeability (junctional complexes) were differentially methylated and expressed between salinity conditions. Expression of genes involved in mitochondrial metabolism was increased as well as the expression of DNA methyltransferases 3a. This study reveals novel aspects on the link of DNA methylation and gene expression patterns.

Background and objective: The incidence of allergic diseases remains high, and many studies have focused on the association between food diversity in infancy and allergic diseases later in life, but their conclusions are still controversial. We aimed to synthesize the literature on the association between childhood diet diversity and atopic diseases. Methods: We searched the PubMed, Cochrane Library, Embase, Scopus, VPCS, and Wanfang databases for studies about food diversity and atopic disease. Seventeen high-quality studies, 15 cohort studies and 2 case‒control studies, were included from 5244 studies with sample sizes ranging from 257 to 5225. Results: All high-quality cohort studies showed that increasing food diversity in infancy can effectively prevent the occurrence of food allergies (6/6). Moderate evidence showed that increased food diversity reduced the risk of asthma (4/7), food sensitization (3/6) and atopic dermatitis (3/5). However, its effect on eczema (7), allergic rhinitis (4), and other diseases remains controversial. Conclusions: Increasing food diversity during infancy is a potential method for preventing food allergies, asthma, atopic dermatitis and food sensitization later in life. There is little or no comparative evidence about the protective effect of food diversity on other atopic diseases.
