In developing nations such as Pakistan, mental health disorders are common. The high incidence might be attributed, in part, to the inadequate understanding of mental health concerns and the low literacy rate. That might be the cause of Pakistan's extremely low number of psychiatrists. Familial hypercholesterolemia (FH) is the most common metabolic disorder and is inherited in an autosomal dominant fashion. FH presents with xanthomas and early coronary artery disease, owing to relatively high levels of low-density lipid-cholesterol (LDL-C) compared to hypercholesterolemia without a genetic predisposition. The first clinical manifestation of this condition could be an acute coronary syndrome in undiagnosed and unsuspecting patient. Various clinical criteria are available to make a diagnosis of FH and a clinician should use the one which he is familiar with. Owing to very high LDL-C levels patients often need multiple oral lipid lowering agents to achieve the desired LDL-C goal. There are multiple novel parenteral lipid lowering agents in offing and some are in pipeline with advantage of high potency and weekly or monthly dosing. This case report emphasizes the importance of FH screening and aggressive treatment to decrease morbidity and mortality in the general population.

Viruses have consistently posed a significant threat to global health, varying in their effect from mild asymptomatic cases to full-blown incapacitating illnesses. India, a country with a tremendous population of over 1.3 billion, encounters unique challenges when managing public health crises, such as the recent re-emergence of the deadly zoonotic Nipah virus (NiV) in Kerala. Our commentary highlights the history of NiV outbreaks in light of the two primary pathways of virus transmission, animal-to-human and human-to-human. We discuss common presenting symptoms of NiV and risk factors contributing to its spread, proposing possible measures to tackle the current challenges in timely diagnosis and infection control. Owing to its high mutation and mortality rate, the NiV outbreak could have potentially transitioned into the next global pandemic, which calls for the implementation of stringent policies and infective protocols by the government and health authorities to curb any future outbreaks.

Objective: Biliary Tract Cancer (BTC) is a significant health concern with limited resectable cases. This meta-analysis assessed the safety of triplet regimens for advanced BTC, specifically gemcitabine, cisplatin, nab-paclitaxel, and gemcitabine, oxaliplatin, erlotinib, along with their associated adverse events. Method: Following PRISMA guidelines, we systematically reviewed PubMed/Medline, Google Scholar, and the Cochrane Library from inception to November 2022. Results: Our analysis of 5 studies favored Gem+Cisplatin+nab-Paclitaxel over GEMOX-T in terms of safety and adverse events. Qualitative analysis of 7 studies (491 patients) showed that Gem+Cisplatin+nab-Paclitaxel also outperformed in median overall survival, progression-free survival, and response rates. Conclusion: This study suggests that Gem+Cisplatin+nab-Paclitaxel is a safer and more effective treatment than GEMOX-T for advanced BTC. It exhibits superior outcomes in terms of survival and response rates. Further research should focus on optimizing treatment regimens to reduce adverse effects in advanced BTC patients.
