Aquagenic Palmar Wrinkling is a rare, transient disorder induced by brief contact with water. It has been noted in cystic fibrosis patients. We describe a unique case of a 17-year-old male with no history of cystic fibrosis, who presented with shriveled palms and multiple whitish papules following brief water immersion.

Background. Population-level estimates of hospitalisations for physical disease in recently treated populations of childhood cancer survivors are limited. In the Australian context, the long-term effects of childhood cancer on survivors and the health system are unclear. We examined the trends in primary hospitalisations for physical disease in a whole-population cohort of 5-year childhood cancer survivors (CCS) diagnosed in Western Australia (WA) from 1982-2014. Methods. We examined hospitalisation records for 2938 CCS and 24 792 matched ‘non-CCS’ controls, discharged from 1987-2019. Andersen-Gill Cox regression models for recurrent events were used to examine hospitalisation risk. Mean cumulative counts were used to examine hospitalisation burden by time since diagnosis and attained age. Negative binomial regression models were used to examine the annual percentage change (APC) in hospitalisations. Results. We identified a higher risk of hospitalisation for all-cause (HR=1.8, 95%CI 1.6-2.0) physical disease in CCS than controls. Compared to controls, hospitalisation risk was highest for second malignant neoplasms (HR=13.2, 95%CI 9.9-17.6) and blood diseases (HR=5.6, 95%CI 1.8-17.1) in CCS. Characteristics associated with higher hospitalisation rates included female gender, diagnosis with malignant bone tumours, cancer diagnosis age between 5-9 years, multiple childhood cancer diagnoses, higher comorbidity, high socio-economic deprivation, and geographic remoteness. The APC in hospitalisations differed between groups (CCS APC=-3.8%; controls APC=2.4%, p<0.05). Conclusions. A higher risk of hospitalisation for physical disease was observed in CCS compared with children not diagnosed with cancer, with the risk continuing to increase up to 30 years post-diagnosis. These findings emphasise the need for continued specialised care and additional research to understand the unmet needs in this population.

Objective:  Anaplastic Lymphoma Kinase ( ALK) gene gain-of-function point mutation leading to its overexpression has recently been identified and targeted in neuroblastoma (NB). We evaluated ALK gene mutation and its protein expression in cases of NB on fine needle aspiration biopsy (FNAB). Material and Methods: FNAB diagnosed cases of NB (n=56) were evaluated with cell blocks for MYCN amplification and ALK protein expression by Fluorescence in-situ hybridization and immunocytochemistry respectively. MGG stained smears (n=22) were used for Next generation sequencing (NGS) analysis using the Cancer Hotspot panel (version2) on Ion Torrent S5 platform. Staging and risk assignment as per International Neuroblastoma Risk Group (INRG) was performed and managed. All the parameters were correlated with overall survival. Results: ALK protein showed cytoplasmic expression in 65% of cases and did not correlate with MYCN amplification (p=0.35), INRG groups (p=0.52), and overall survival (p=0.2); however, ALK+ poorly differentiated NB showed better prognosis (p=0.02). ALK negativity was associated with poor outcome by Cox proportional hazard model (hazard ratio=2.36). ALK gene, exon 23 missense mutations (F1174L) were seen in 2/21 cases with an allele frequency of 8% and 54%. Both these cases showed ALK protein expression and died of disease within 1 and 17 months respectively. A novel IDH1 exon 4 mutation was also detected. Conclusion:  ALK expression is a promising prognostic as well as a predictive marker in advanced NB along with traditional prognostic parameters. FNAB smears are suitable for NGS and ALK gene mutation confers a poor prognosis.

Rapid influenza diagnostic tests (RIDTs) have variable sensitivity. In a community-based population of kindergarten through 12th grade (K-12) students, we assessed factors that may influence RIDT performance using 2,368 paired results from Sofia® Influenza A + B fluorescent immunoassay and reverse transcription polymerase chain reaction (RT-PCR). RIDT sensitivity and specificity were 76.1% (95% CI: 72.8—79.1) and 97.2% (96.2—97.9), respectively. Factors associated with sensitivity included runny nose (OR=3.0, p<0.001), nasal congestion (1.59, p=0.045), days from symptom onset (per day; 0.75; p<0.001), myalgia (0.61; p=0.014), age (per 5 years; 0.55; p=0.001), and detection of another virus (0.50; p=0.043). Understanding these factors can aid in interpreting negative results.

A 60-year-old woman with breast invasive ductal carcinoma underwent FDG PET/CT scan for cancer staging. A synchronous colon cancer was detected incidentally. After resection of colon cancer, she received CCRT for breast cancer. Marked therapeutic response was found throughout the follow-up period. Also, there was no evidence of cancer recurrence.

Counterparty sentiment and major uncertain factors always affect credit risk contagion in credit risk transfer (CRT) market. In order to effectively analyze credit risk contagion, this paper proposes a stochastic credit risk contagion model considering sentiments, in which the stochasticity is driven by Lévy process. Based on the Lyapunov function method, sufficient condition of finite-time stability is derived. The impacts of counterparty sentiment and Lévy process on credit risk contagion are analyzed. To effectively control credit risk contagion, the stochastic credit risk control model is established, and the optimal control strategy is given by using Pontryagin’s maximum principle. Numerical simulating results show the correctness and effectiveness of the theory.

In this work, we obtain new sufficient conditions for the non-oscillatory solutions of forced nabla fractional difference equations with positive and negative terms. The results are developed in sense of Caputo nabla fractional difference operator and by the help of Young's inequality as well as an equivalent representation in form of a Volterra-type summation equation. Geometrically, a complex dynamical network (CDN) can be regarded as the interconnected system composed of the node subsystem (NS) and the link subsystem (LS) coupled with each other. Guided by this idea, in order to achieve the goal of each node following asymptotically its own reference target in a CDN, this paper investigates the model following adaptive control (MFAC) problem of NS via the dynamics of links, which implies that the LS plays the important dynamic auxiliary role in the MFAC realization of nodes. Meanwhile, we focus on the condition that the links state information is unavailable, due to sensor practical application and measurement cost constraints. To obviate this restriction, we construct the asymptotical state observer for the LS. Next, to achieve the control goal of this paper, an appropriate Lyapunov candidate function is selected, by which the MFAC scheme for NS is synthesized based on the state observer of LS. Finally, the simulation example is performed to demonstrate the theoretical results in this paper.

In this essay, the nonlinear fractional integral equation is studied. Akbari-Ganji’s Method (AGM), Homotopy Perturbation Method (HPM) and Vibrational Iteration Method (VIM) are applied to obtain its solution. We present a new strategy for finding the approximate solutions to Fractional differential equations. We experience Fractional differential equations, which are broadly utilized in fluids. In this article, we have used analytical methods to check the correctness of the answers. Ordinary equations and fractional differential equations are related to entropy and wavelets, and so on. A few examples are employed to appear accurate and simple to implement and demonstrate the method. The solutions are clarified in convergent series. Some well-known models for anticipating the oscillation behavior of the action in a vibrating system are presented, then with the help of fractional calculus which could exceptionally powerful tool in mathematics and modeling of complex systems, a model for the same system is proposed. Compare the models and finally show that the proposed fractional model not only includes non-fractional models but also predicts the behavior of the system more comprehensively.

For the problem of influence of background level on estimating target spatial azimuth distribution value, a method of MVDR spatial spectrum estimation based on Richardson-Lucy is proposed. The output results of MVDR spatial spectrum estimation method were deconvolution with point scattering function and Richardson-Lucy iterative method of image restoration, and the background level and its influence on estimating target spatial azimuth distribution value was reduced. The results of measured data show that, this method is a post-processing method, inherits the high resolution performance of MVDR spatial spectrum estimation method, has more sharp peak value, when signal to noise ratio is lower, has better performance of estimating target spatial azimuth distribution value.

The authors performed a detailed retrospective analysis of diabetic patients undergoing isolated coronary artery bypass graft surgery (CABG) aiming to investigate the association of the preoperative Glycosylated hemoglobin with occurance of postoperative atrial fibrillation. Altough statistical analysis showed a weak relationship between HbA1c values of 9.06% or above and PoAF, they concluded that serum level of HbA1c could not be used as a predictor for the development of PoAF. But there are many questions to be asked and answers to be found.

Background: Interleukin 33, Fetuin A and Cytokeratin 18 are important inflammatory mediators in coronary artery diseases; however, although the plasma levels of these markers are generally used for the evaluation, their pericardial levels have not been explored. This study aims to determine the pericardial behavior of these mediators in coronary artery diseases and their regression to plasma. Methods: The gene expression and protein levels of interleukin 33, Fetuin A and Cytokeratin 18 in plasma and pericardial fluid were investigated in 40 patients undergone coronary bypass surgery. Results: According to ELISA findings, Interleukin 33 and Fetuin A protein levels have been found to be significantly increased in pericardial fluid compared to plasma, p<.05. Although cytokeratin 18 protein level was also dramatically increased in pericardial fluid, it was not statistically significant. Gene expressions of interleukin 33 and Fetuin A were found to be significantly increased in pericardial fluid compared to plasma, corroborating the protein levels. Conclusions: Interleukin 33, Fetuin A and Cytokeratin 18 levels are clinically important and biomarkers that should be followed. Fluctuations of these biomolecules suggest that pericardial fluid has a potential to be used as a source of biomarkers for coronary artery diseases, even if sampling from the pericardium is not sustainable. Increased Interleukin 33 and Fetuin A proposes apoptotic machinery activated in the heart tissues of our patients. Pericardial fluid is an important target for both diagnosis and treatment because of its proximity and relationship to the heart tissue.

Background: Four factor prothrombin complex (PCC4), a concentrate of factors II, VII, IX, X and protein C and S, has been used selectively for reversal of oral anticoagulation prior to surgery.  There is data to support PCC4 as opposed to supplemental fresh frozen plasma (FFP) to manage postoperative bleeding following cardiac surgery.  The preemptive, intraoperative use of PCC4 in cardiothoracic surgery has not been studied though it may prevent postoperative bleeding, the need for blood transfusion and the risk of transfusion related acute lung injury, volume overload, and right ventricular (RV) heart failure. The purpose of this study is to evaluate the intraoperative administration of PCC4 to decrease bleeding and lower the rate of blood transfusion. Methods: A single institution retrospective chart review conducted from May 2020 to November 2021 of patients who received PCC4 intraoperatively during cardiothoracic surgery of high risk variety. Patients were evaluated for type of surgery, demographics, baseline anticoagulation, PCC4 dose, type and quantity of blood transfusion within 72 hours postoperatively, chest tube output, incidence of right ventricular failure, hypersensitivity reactions, acute kidney injury, thrombosis, acute lung injury, and mortality within 45 days of the operative dose of PCC4. Results: Thirty five patients received PCC4 at a mean dose of 2920 units. Sixty five percent of cases were LVAD or heart transplant. The protocol is to use PCC4 30 units/kilogram immediately after completion of protamine administration. Inclusion criteria are: cardiothoracic surgery with increased risk of postoperative right heart failure commonly secondary to blood product transfusion, or cardiothoracic surgery associated with increased risk of bleeding, including: heart transplant, LVAD implant, aortic dissection, and redo sternotomy (e.g. coronary artery bypass). Total chest tube output was recorded as a mean of 757 mL for 24 hours after surgery (32 ml/hr). Overall median event rates of fresh frozen plasma (FFP) and red blood cell (RBC) transfusion were 0 (interquartile range 0 - 3 units) and 4 (interquartile range 2-5 units). Overall, forty-three percent and eighty-nine percent of cases received FFP and RBC, respectively.  There was one occurrence of right ventricular failure, one occurrence of acute kidney injury requiring renal replacement therapy, one occurrence of venoarterial extracorporeal membrane oxygenation, one occurrence of venous thromboembolism related to a central venous access line, and one death unrelated to surgery or PCC4 that was attributed to advanced heart failure not amenable to advanced therapies. Conclusion: Overall patients received a low rate of blood transfusion, had minimal chest tube output, and there was a small incidence of right heart failure. Patients did not have an increased risk of adverse effects such as acute kidney injury or venous thromboembolism. A randomized controlled clinical trial comparing the observed dose and timing of PCC4 versus routine postoperative bleeding management with blood product transfusion is recommended.

Due to the barcode-based billing method, a significant throng on the supermarket during discount bids or weekends finds themselves in long lineups.Instead of waiting long time in a queue, the proposed method is to let the client process products directly and charge them in the trolley. After a quick scan in the carriage, customers need to add the product and when they are finished, the amount is shown in the carriage. In addition, the client can log in to the app that shows the list and amount of all the added products. The customer can pay digitally via the application once done. Thus the time spent elsewhere in long queues is reduced and they are relieved of the tediosity with bar codes scanned.

We consider the equations of motion of a bar, with given density, infinite in both directions, subjected to longitudinal vibrations under the action of an external load, and a stress-strain relation represented by a fractional order operator. Using three types of fractional operators, the initial-boundary value problems associated with the described phenomenon are posed and solved. Through the bivariate Mittag-Leffer function, which has been recently introduced, we find the fundamental solution of these problems and calculate their moments.

Background: Midurethral sling surgery is the current gold standard worldwide for stress urinary incontinence (SUI) surgery. However, concerns surround mesh-related adverse events associated with the midurethral sling. The decision to use the midurethral sling for surgical treatment has become a challenging one for clinicians, surgeons and patients. Objectives: We sought to determine the factors for 5-year complications after midurethral sling surgery, to improve the clinical decision-making process. Study Design: Records were reviewed from a total of 1,961 female patients who underwent their first midurethral sling surgery for SUI between 2003 and 2018 at a single teaching hospital in Taiwan. A multivariable Cox proportional hazard model calculated the hazard ratios of risk factors for surgical complications, after adjusting for confounders. Results: Surgical complications (i.e., secondary surgery and urinary retention) occurred in 93 (4.7%) patients within 5 years following the index operations. These patients were more likely to be older, to have a history of menopausal syndrome within 1 year prior to the index operation,hormone replacement therapy (HRT), slower average flow rate, and longer voiding time compared with patients without surgical complications. In the multivariate analysis, menopausal syndrome or HRT (adjusted hazard ratio, 1.794; 95% confidence interval, 1.055–3.050, p=0.0390) was significantly associated with surgical complications at 5 years, after adjusting for age, gender, diabetes, average flow rate, and sling type. Conclusion: Our findings suggest that a history of menopausal syndrome within 1 year prior to surgery or a medication history of HRT may be risk factors associated with surgical complications

Objective To investigate the relationship between serum fructose and uric acid levels in patients with polycystic ovary syndrome (PCOS). Design A case-control study. Setting University-affiliated in vitro fertilization clinic. Population 292 patients with PCOS and 482 controls. Main Outcome Measures Serum fructose, uric acid and metabolic measurements. Results Compared with controls, serum fructose and uric acid levels were significantly increased in women with PCOS and patients with PCOS accompanied by metabolic disorders exhibited higher serum fructose and uric acid levels (P < 0.001). Restricted cubic splines indicated that serum uric acid levels linearly and positively correlated with serum fructose levels in women with PCOS (Poverall < 0.001, Pnon-linear = 0.30), whereas no correlation was found in controls (Poverall = 0.712, Pnon-linear = 0.43). Additionally, even after adjusting for confounding factors, serum fructose levels were an independent risk factor for hyperuricemia in patients with PCOS (P = 0.001; odds ratio, 1.380; 95% confidence interval, 1.207–1.577). Conclusions There was a significantly positive association of elevated uric acid levels with serum fructose levels in PCOS and was closely correlated with PCOS-related metabolic disorders, highlighting the importance of further research into the biological mechanisms of fructose and uric acid in the development of PCOS. In order to balance the relationship between the protection of personal information and the use of personal information, the Personal Information Protection Law has set anonymization standards. Although, technically, absolute anonymity is not achievable, but the legal goals are not the same as the technical goals. In order to maximize social welfare, it is necessary to reduce the quality of anonymization to a certain extent to ensure the universality and speed of the personal information governance program. Therefore, it becomes feasible to realize the anonymization of personal information in law. In order to facilitate the anonymization of judgments, different jurisdictions have established different rational people standards under the guidance of value. The EU standard overemphasizes the protection of the rights of information subjects, which is not conducive to the use of personal information, and it is difficult to maximize social benefits. The Anglo-American standard takes into account both “security” and “efficiency”, while protecting the information subject, while leaving enough space for personal information processors. Background and Purpose: Blood-brain barrier (BBB) breakdown is one of the most crucial pathological changes of cerebral ischemia-reperfusion (I/R) injury. Trilobatin (TLB), a naturally occurring food additive, exerts neuroprotective effect against cerebral I/R injury as demonstrated in our previous study. This study was designed to investigate the effect of TLB on disruption of BBB after cerebral I/R injury. Experimental Approach: Rats with focal cerebral ischemia caused by transient middle cerebral artery occlusion (MCAO) and brain microvascular endothelial cells along with human astrocytes to mimic blood brain barrier (BBB) injury caused by oxygen and glucose deprivation (OGD) followed by reoxygenation (OGD/R). Key results: The results showed that TLB effectively maintained the integrity of BBB and inhibited neuronal loss following cerebral I/R challenge. Furthermore, TLB dramatically increased tight junction proteins including ZO-1, occludin and claudin 5, as well as decreased the levels of apolipoprotein E (APOE) 4, cyclophilin A (CypA), and phosphorylated nuclear factor kappa B (NF-κB), thereby reduced proinflammatory cytokines. In addition, TLB also decreased Bax/Bcl-2 ratio and cleaved-caspase 3 level along with reduced the number of apoptotic neurons. Intriguingly, molecular docking and transcriptomics predicted MMP9 was a prominent gene evoked by TLB treatment. Furthermore, the protective effect of TLB on OGD/R-induced the loss of BBB integrity in human brain microvascular endothelial cell and astrocyte co-cultures in vitro was markedly reinforced by knockdown of MMP9. Conclusions and implications: Our findings reveal a novel property of TLB: saving BBB disruption following cerebral I/R via targeting MMP9 and inhibiting APOE4/CypA/NF-κB axis.

Background and Purpose: Cytokine storm invoked during acute and chronic lung injury promotes alveolar damage and remodeling. The current study shows that degraded elastin-targeted nanoparticles releasing doxycycline (Doxy NPs) are potent in mitigating cytokines storm, migration of immune cells in the lungs, and inhibiting inflammasome pathways in the LPS mouse model. Experimental Approach: Cytokine storm and lung injury were induced using LPS and elastase in C57BL/6 mice (rodent model for emphysema). The mice were then treated with I.V. Doxy NPs, blank NPs, or Doxy a day before LPS administration. Cytokine levels, immune cell population, and MMP activity were analyzed in BALF 4 hours after LPS administration. Additionally, gene expression of IL-6, IL-1beta, MCP-1, NLRP3, Caspase 1, and MMPs were investigated in alveolar cells on day three after LPS administration. Key Results: Doxycycline NPs but not Doxycycline significantly decreased IL-6, TNF-α, and IL-23 and were significantly more effective in decreasing the percentage of immune cells in the BALF. This is the first in-vivo study to demonstrate that Doxycycline can effectively inhibit lung inflammasome pathways. Conclusion and Implications: IV administration of elastin antibody conjugated Doxycycline-loaded albumin Nps can effectively modulate the local immune environment in the lungs, which is not achieved by IV Doxycycline even at a 100-fold higher dose. This novel method of drug delivery can effectively lead to the repurposing of traditional Doxycycline as a potential adjunct treatment for managing the cytokine storm in the lungs in COPD and viral infections.

Resource fluctuation is a major driver of animal movement, influencing strategic choices such as residency vs nomadism, or social dynamics. The Arctic tundra is characterized by strong seasonality: resources are abundant during the short summers but scarce in winters. Therefore, expansion of boreal-forest species onto the tundra raises questions on how they cope with winter-resource scarcity. We examined a recent incursion by red foxes (Vulpes vulpes) onto the coastal tundra of western Hudson Bay, an area historically occupied by Arctic foxes (Vulpes lagopus) that lacks access to anthropogenic foods, and compared seasonal shifts in space use of the two species. We used 4 years of telemetry data following 8 red foxes and 11 Arctic foxes to test the hypothesis that the movement strategies of both species are primarily driven by temporal variability of resources. We also predicted that the harsh tundra conditions in winter affect red foxes more than Arctic foxes, which are adapted to this environment. Dispersal was the most frequent winter movement strategy in both fox species, despite its association with high mortality (winter mortality was 9.4 times higher in dispersers than residents). Red foxes consistently dispersed towards the boreal forest, whereas Arctic foxes primarily used sea ice to disperse. Home range size of red and Arctic foxes did not differ in summer, but resident red foxes substantially increased their home range size in winter, whereas home range size of resident Arctic foxes did not change seasonally. As climate changes, abiotic constraints on some species may relax, but associated declines in prey communities may lead to local extirpation of many predators, notably by favoring dispersal during resource scarcity.
