Background: Locally recurrent papillary carcinoma of thyroid is a treatable disease. Tracheal infiltration is associated with impaired tumor free survival and increased disease specific mortality. Advanced surgical technique has now allowed tracheal reconstruction with extended tracheal resection at the same time preserving vocal cord function and increasing survival. Case presentation: A 46-year female patient with a history of total thyroidectomy + bilateral neck dissection for carcinoma thyroid two years back presented with recurrent thyroid mass in neck with endotracheal invasion. This was successfully treated with segmental tracheal resection with end-to-end anastomosis after 4 cm defect. Post – operative course was uneventful. Conclusion: Endotracheal invasion of recurrent carcinoma thyroid is not a contraindication for surgery. Full circumferential resection and end to end anastomosis is preferred to shaving trachea. It can be safely anastomosed up to 5 cm defect length. Apart from giving immediate relief of intratracheal bleeding and obstructive airways, it gives long-term disease-free survival.

Blockchain technology is a distributed ledger digital system that records all transactions on a network of interconnected systems in a way that is impossible to edit or hack. Thoughts of cryptocurrency pop into mind when blockchain is mentioned, but blockchain holds vast possibilities in other facets such as big data analytics, research, the Internet of things, healthcare etc. Blockchain technology has many practical applications in the healthcare sector, including precision medicine and interoperability, supply chain management, exchange of health information, security and authorisation, all of which are pivotal for effective health systems and quality health services. Inadequate infrastructure, lack of funding, computer literacy, and poor government policies have been the significant hindrances. The challenge of establishing blockchain technology must be addressed. Exploring its architecture in the Estonian healthcare-based system, the possibility of its deployment in the Nigerian health sector is feasible with the right approach and government policies. By discussing the potential benefits of blockchain in the health sector, the obstacles that impede the adoption of this technology and the recommendations to overcome them, this perspective piece aims to create awareness and close the wide gap between this idea and its implementation in our healthcare system. This will improve patient outcomes and strengthen our healthcare planning and management system.

This paper investigates the problem of unmanned aerial vehicle (UAV) aerial refueling docking control subject to system model uncertainties and multiple disturbances. The control of aerial refueling docking can be regarded as the problem of precise trajectory tracking of probe-drogue refueling (PDR) under various disturbances. Different from the traditional backstepping control, adaptive dynamic surface control (ADSC) avoids the “differential expansion” in the derivation process of virtual control, which greatly simplifies the difficulty of controller design. Then, a novel ADSC strategy based on radial basis function neural network (RBFNN) is proposed to improve the anti-disturbance performance and docking control accuracy of PDR system. Finally, the effectiveness of the proposed control method is verified by digital simulation.

The Eyring-Powell non-Newtonian fluid model is widely used to described flow behavior with lowest and highest shear rates. Statistical analysis is carried out in this present article to explore non-Newtonian BL flow past an elongating sheet which is moving with linear velocity. Similarity transforms are applied to express fluid governing equations as ODEs which are then solved by bvp4c algorithm in MATLAB software. In the energy equation thermal radiation and joule heating effects are considered. Important key factors such as Nusselt number and friction at the surface are computed with respect to different influencing fluid parameters.

Duchenne muscular dystrophy is an incurable X linked recessive genetic disease caused by mutations in the dystrophin gene. Many researchers aimed at restoring truncated dystrophin via viral vectors. But the low package capacity and short duration of vectors hampered their clinical application. For these reasons, we constructed four lentiviral vectors, which contained truncated and sequence-optimized dystrophin genes driven by muscle specific promoter. The four lentiviral vectors stably expressed mini-dystrophin in C2C12 muscle cells in vitro. To estimate the treatment effect in vivo, we transferred the lentiviral vectors into neonatal C57BL/10ScSn- Dmdmdx mice through the local injection. The level of modified dystrophin expression increased, while the distribution of that also restored in treated mice. At the same time they showed the restoration of pull force and the decrease in the number of mononuclear cells. The remissions lasted three to six months in vivo. Moreover, no integration sites of vectors distributed into the oncogenes. In summary, this study has preliminarily demonstrated the feasibility and safety of mini-dystrophin gene carried by lentiviral vector for DMD gene therapy, and provided new strategy to restore truncated dystrophin.

Background: Here we conducted this study to find the changes in the hematological parameters of healthy individuals that can predict their immune status, also this study aims to find out whether herd immunity against COVID-19 is developing or not in the Pakistani population. Methods: A comparative cross-sectional study of 210 healthy individuals was conducted. All individuals were divided into three groups i.e IgG positive, IgG negative, and IgG and IgM both positive based on ELISA results. Data analysis was done by SPSS version 25 for windows. Results: A statistically significant effect was found among the three groups in terms of mean hemoglobin level, mean hematocrit, mean corpuscular hemoglobin concentration, mean red blood cells count, RDW-CV, mean lymphocyte, mean neutrophil, mean eosinophils, and mean basophil. The study also showed that 52.8% (n=74) had neither taken vaccination nor had any history of previous covid infection but still their antibodies were positive. Conclusion: There was a statistically significant difference among hematological parameters between immune and nonimmune groups and can predict the covid immune status also the study predicted that herd immunity against COVID-19 is developing in Pakistan.

Background: Many view attrition as one of the biggest barriers to effective delivery of cART in resource-limited settings in sub-Saharan Africa (SSA). In this study, our objective was to describe the incidence and predictors of attrition among adults enrolled in cART programs in two referral hospitals in the northern coastal areas of Eritrea. Methods: This was a retrospective review of patient records of 464 patients [Male: 149(35.6%) vs. Females: 269(64.4%)] aged 18 years who initiated cART between 2005 and 2021. The main outcome measures were attrition (loss-to-follow-up (LTFU) plus mortality) and associated outcomes. Kaplan-Meier statistics were used to evaluate survival probability of attrition. Independent predictors of attrition were evaluated using a multivariable Cox proportional hazard model. Results: A total of 418 patients [Male: 149(35.6%) vs. Female: 269 (64.4%)] were studied. At baseline, the mean (±SD) age (SD) was 34(±11.2) years; median (±IQR) CD4 + T-cell count was 151 (IQR: 87-257) cells/µL. After a follow-up time of 39,883 months, 127 ((30.4%), 95% CI [26-35]) attrition events were reported, translating into a cumulative incidence of 2.9/1000(2.4-3.5) per 1,000 people-months (PMs) were reported. During the same period, 97 (23.11%) patients died, 32(7.7%) were LTFU, and 47(11.2%) transferred out. In the adjusted multivariate Cox regression model, an increased risk of attrition was associated with the year of enrollment (aHR = 1.07, 95% CI 1.00-1.15, p-value = 0.04); ethnicity (Afar: aHR=3.21, 95% CI: 1.84-5.59, p value < 0.001) (Others: aHR = 2.67, 95% CI: 1.14-6.25, p value = 0.024) and cART backbone: (TDF+FTC: aHR=2, 95% CI: 1.21-3.32, p value = 0.007). On the contrary, the risk of attrition decreased per unit increase in baseline CD4 + T-cells/μL (uHR=0.998, 95% CI 0.996-0.999, p-value<0.001). Conclusion: Despite expanded treatment and decentralization of cART programs, mortality due to advanced disease at enrollment remains high in peripheral settings. A concerted effort is required to reduce late enrollment and improve the management of patients with advanced disease in decentralized programs.

Cell-free massive multiple-input multiple-output (CFMM) network is projected as the latest technology for the fifth-generation and beyond wireless networks. The recent research trend is to extensively study and analyse CFMM network for its advantages and bottlenecks. The CFMM network is strongly affected by pilot contamination (PC) which is one of the bottlenecks due to which quality of service (QoS) and accuracy of channel estimation gets impacted. Therefore, we address this problem by presenting a novel pilot assignment algorithm to mitigate PC and deep learning aided channel estimation for reducing channel estimation error for the CFMM systems to maximize spectral efficiency. We derive achievable UL and DL spectral efficiency (SE) expressions for the proposed system, and compared with Minimum Mean Square Error(MMSE) and Maximum Ratio (MR) combining techniques. The performance of cellular massive MIMO is derived for comparison. For the same cellular set up,the proposed CFMM system achieves higher SE than the cellular massive MIMO. Numerical results prove the efficacy of the proposed CFMM system to some of the existing schemes in this domain.

Human health is having a severe threat by bacterial infection. As it attracted researchers to work and synthesize antibacterial agents. This work develops Isatin-1,2,3-triazole as an antibacterial agent using one-pot synthesis i.e., click chemistry. In-vitro antibacterial activity of Isatin-1,2,3-triazole was evaluated against common pathogens, Gram-negative (Escherichia coli) and Gram-positive (Staphylococcus aureus) pathogens by microdilution method as well as well-diffusion method. The results showed that Triazole has enhanced antibacterial activity of Isatin against E. coli, and S. aureus. Structure-activity analysis and molecular docking studies revealed that the antibacterial activity of Triazole-tethered Isatin tosyl azide is particularly better as compared to N-propargyl Isatin. Furthermore, Isatin triazole showed best binding affinity with Staphylococcus aureus (PDB ID:4TU5) and E. coli (PDB ID:6YD9) bacteria proteins which are -10.44KJ/mol and -8.4KJ/mol. Isatin triazole showed high GI absorption and low toxicity parameters and its satisfied Lipinski’s rule of five and passed Veber, and Ghose rules. It’s also maintained the stability with the proteins during md simulation for 100 ns. Overall, the study found that these Isatin triazole molecules have considerable therapeutic qualities, and their in-silico analysis strongly suggests that more clinical research can be conducted to gain insight into the mechanisms of action in healing a range of bacterial diseases.

This study proposes a novel 24-bit delta sigma analogue-to-digital converter (ADC) for an audio system-on-chip (SOC) with a fully differential switched-capacitor preamplifier and a capacitor-array DAC feedback structure. This system reduces the equivalent input noise and overcomes the nonlinearity and circuit complexity caused by the multi-bit quantiser, This structure also improves the accuracy of the quantiser and achieves zero static-power consumption. Combined with the capacitor array DAC feedback structure, the entire system does not require additional reference voltage, which reduces power consumption and is more conducive to system integration on the chip simultaneously. Finally, the design achieves a maximum signal-to-noise ratio (SNR) of 120 dB and a low static power consumption of 5 mW at a sampling rate of 24.576 MHz.

In the present work, we focus on the longitudinal model of microtubules (MTs) proposed by Satar i c ̵́ et al. [Phy. Rev. E 48, 89 (1993)], that consider cell MTs to have ferroelectric properties, i.e., a displacive ferro-distortive system of dimers and usually referred to as u-model of MTs. It has been shown that during the hydrolysis of GTP into GDP, the energy released is transferred along the MTs trough kink-like solitons. Substantially, we propose to theoretically investigate the dynamic of MTs by intrinsically taking into account the effect of the oriented molecules of polarized cytoplasmic water and enzymes surrounding the MT. In this regards, we introduce a cubic nonlinear term in the electric potential characterizing the polyelectrolyte features of MTs and show that in addition to the kink and antikink solitons, asymmetrical bright and dark solitons, and discrete modes can also propagate along the MTs. Theses results are supported by numerical analysis. The investigation shows us that the nonlinear dynamics of MTs is strongly impacted by the intrinsic electric field, the polyelectrolyte and the viscosity effects. Moreover, new solitons and discrete solitary modes may help to find new phenomena occurring in the microtubulin systems.

Cereal rust diseases are caused by Puccinia spp. urediniospores that travel by air currents to cause losses in wheat, oat, barley, and rye. In western Canada, inoculum can arrive from the USA through the Great Plains or via a Pacific corridor west of the Rocky Mountains. Moreover, climate and land use are known factors in Puccinia dissemination, but their relative effects remain poorly understood. We investigated aeromycobiota in western Canada by weekly air-sampling over four growing seasons (2015-2018) three mixed-crop sites in British Columbia (BC) and one season (2018) at five sites in southern Alberta (AB). ITS2-based metabarcoding and novel bioinformatic comparative analyses to known, especially curated cereal rust fungal sequences, was used for species identification. The overall aeromycobiota and rust fungal community diversity was higher west than east of the Canadian Rockies. This mountain range delineates climate and land use and also creates a barrier to wind flow to prevent the spread of rusts and other plant pathogens (e.g., Bipolaris, Blumeria). We recovered seven major cereal rusts and revealed their geographic and seasonal dynamics at the eight sampling sites. Forward and reverse trajectory HYSPLIT model simulations predicted the potential sources of rust urediniospores and their pathways of movement modulated by air currents, through which some pronounced changes in the abundance of wheat rust pathogens were explained. This study paves the way for a potential application for pathogen monitoring and disease risk forecasting as part of the design and development of an early-warning system for enhanced biovigilance against crop diseases.

In this article, the stability and stabilization problems of saturated impulsive nonlinear control systems are investigated. With the use of a class of clock-dependent Lyapunov functions and polytopic representation approach, new sufficient conditions ensuring the local exponential stability (LES) are established in the framework of dwell time, which allow that both the continuous and discrete parts of the systems are destabilizing at the same time. Moreover, based on the sum of squares programming, an optimization algorithm is proposed to design the saturated impulsive controller with improvement of the allowable impulsive dwell-time and the size of the domain of attraction. Finally, the simulation results demonstrate the effectiveness of the results.

Avian diet can be affected by site-specific variables, such as habitat, as well as intrinsic factors such as sex. This can lead to dietary niche separation, which reduces competition between individuals, as well as impacting how well avian species can adapt to environmental variation. Estimating dietary niche separation is challenging, due largely to difficulties in accurately identifying food taxa consumed. Consequently, there is limited knowledge of the diets of woodland bird species, many of which are undergoing serious population declines. Here, we show the effectiveness of multi-marker faecal metabarcoding to provide in-depth dietary analysis of a declining passerine, the Hawfinch (Coccothraustes coccothraustes). We collected faecal samples from (n=262) UK Hawfinches prior to, and during the breeding seasons in 2016-2019. We detected 49 and 90 plant and invertebrate taxa, respectively. We found Hawfinch diet varied spatially, as well as between sexes, indicating broad dietary plasticity and the ability of Hawfinches to utilise multiple resources within their foraging environments.

Background: Preterm delivery is a major global health concern that continues to be a primary focus in obstetric and pediatric units in Sub-Saharan African countries. Preterm birth complications are the largest cause of mortality in children under the age of five worldwide and higher in sub-Saharan Africa. The aim of this study was to investigate the magnitude of preterm delivery and its associated factors at Tanzania’s Muhimbili National Hospital, a tertiary hospital. Methodology: An institutional based cross-sectional study was undertaken at Muhimbili National Hospital’s maternity ward in the year 2021.The study enrolled 422 mothers who met the criteria. Mothers were interviewed face to face using a structured questionnaire, other clinical data were collected from medical records and the registry. Chi square (x2) and Fisher exact tests were used to evaluate the associated factors. The strength of association between the independent and dependent variables was determined using a logistic regression model. Results: In this stu the proportion of mothers who had preterm delivery was 19.91%. The formation of secondary cell walls is tightly regulated spatio-temporally by various developmental and environmental signals. Successful fine-tuning of the trade-off between secondary cell wall biosynthesis and stress responses requires better understanding of how plant growth is regulated under environmental stress conditions. However, current understanding of the interplay between environmental signaling and secondary cell wall formation is limited. The lipid-derived plant hormone jasmonate (JA) and its derivatives are important signaling components involved in various physiological processes including plant growth, development, and abiotic/biotic stress response. Recent studies suggest that JA may be involved in secondary cell wall formation. We tested this hypothesis using the transcription factor MYB46, a master switch for secondary wall biosynthesis, and JA treatments. Both the transcripts and protein levels of MYB46 were significantly increased by the JA treatments, which also triggered the upregulation of MYB46 downstream genes with increased secondary wall formation. We then show that this JA-induced upregulation of MYB46 function was mediated by MYC2, a basic helix-loop-helix (bHLH) domain–containing transcription factor, which plays a pivotal role in the JA-mediated changes. We conclude that this MYC2-MYB46 module is a key component of the plant response to JA signaling.

Amoxicillin induced mast cell degranulation mediated by FcεRI specific IgG: a new insight to beta-lactam allergy To the editor,Immediate drug hypersensitivity reactions (IDHR) to beta-lactams (BL), referred to here as beta-lactam allergy (BLA), is defined as the development of symptoms such as urticaria and bronchospasm within one hour of beta-lactam exposure (1). Classical Type 1 immunoglobulin E (IgE)-mediated allergy describes a response which occurs within minutes of allergen exposure in a previously sensitised person. It has therefore been widely accepted that BLA is mediated by beta-lactam specific IgE (BL-sIgE) (1). In other Type 1 IgE driven allergies, such as food allergy, sIgE to major allergen components can be shown to be highly specific (2, 3). However, for BLA, BL-sIgE has poor specificity, suggesting alternative mechanisms might be involved (4).Type 1 IgE mediated reactions are mainly mediated by basophils and mast cells (MCs). Previous studies have demonstrated the use of the basophil activation test (BAT) in predicting BLA, with European allergy guidance stating that BAT can aid BLA diagnosis (5). However, BAT requires fresh whole blood which is not ideal for in depth mechanistic exploration.In an in vitro BLA model using human progenitor derived MCs, we have shown that MCs sensitised with serum from a patient with confirmed BLA, demonstrated amoxicillin (AMX) induced degranulation (CD63+ positivity) after 30 minutes of stimulation with AMX (Figure 1B). In contrast, sensitisation with serum from a non-BLA control demonstrated no response (Figure 1C).The mechanism of degranulation from the BLA model is very different from the classical IgE-mediated Type 1 degranulation such as that seen in food and inhalant allergy (6). In our in vitro model of classical IgE-mediated Type 1 degranulation, sIgE binds to FcɛRI with high affinity which is not disturbed by pipetting and washing (6). In our BLA model, BLA serum sensitized MCs have a much weaker degranulation and can easily lose their response to beta-lactam stimulation under physical disturbance (Figure 1A).We hypothesised that FcɛRI was involved in the degranulation process in BLA, not through the high affinity binding of IgE, but instead through IgG binding. We produced a recombinant fragment representing the extracellular portion of the FcɛRI receptor alpha chain, which is mainly responsible for antibody binding. When added to the serum, this fragment binds and neutralizes FcɛRI-specific antibody—IgE and IgG, by preventing them binding to the cells. BLA serum was treated with increasing concentrations of the FcɛRI fragments and then added to the MCs for sensitization. After stimulating the sensitized MCs using AMX, we observed a dosage-dependent decrease of MCs degranulation, indicating direct involvement of the FcɛRI in the degranulation process of BLA (Figure 2A).To investigate whether degranulation was mediated by IgE, we neutralized serum IgE by adding omalizumab before MC sensitisation. This showed no impact on the AMX induced degranulation (Figure 2A). In contrast, omalizumab inhibited MC degranulation in a classical IgE model, using pooled serum from cat allergic patients stimulated with cat allergen (Figure 2B).To determine whether FcεRI-specific IgG is functionally responsible for the AMX induced MCs degranulation, we isolated IgG from BLA serum using IgG-depletion spin columns. MCs sensitized with the IgG-only fraction showed no response to anti-IgE, but retained a strong response to AMX (Figure 2B). In comparison, in the IgG-depleted serum fraction (~60% IgG removed), we observed a strong degranulation to anti-IgE stimulation but a weakened response to AMX (Figure 2C).A limitation of this preliminary work is it used serum from only one BLA patient.In summary, we developed a cell model for BLA using in vitrodifferentiated human MCs. This BLA model has demonstrated the involvement of FcεRI-specific IgG in mediating AMX-induced MCs degranulation. Non-intrusive load monitoring(NILM) can identify the appliance categories, using time, work duration and the work states through data collected from a smart meter. With NILM technique,people can conduct the power supply and power consumption scientifically and reasonably so as to partially or completely solve the problem of imbalance between power supply and demand. The low-frequency non-invasive load state identification algorithm can identify the load using low-frequency signal without additional hardware facilities, so it has broad development prospects. However, it is difficult to improve the accuracy of low-frequency load identification because there is little information available. In this paper, a low-frequency non-invasive load state identification algorithm combining electrical switch state classification LSTM algorithm and maximum likelihood probability algorithm based on recursive reasoning is proposed. The load identification is divided into two steps, which reduces the overall difficulty and improves the accuracy of state identification. The experimental results on Ampsd2 data set show that the proposed algorithm can achieve better results than other NILM algorithms.

In response to the COVID-19 pandemic shut-down of in-person research laboratories, remote data collection became mainstream in developmental psychology research. The aim of this study was to test the feasibility of two remote data collection methods for studying sleep and motor development in infancy and the relationship between the two. We asked 1,371 parents of infants aged 4 to 17 months who used the Nanit baby monitor to take the on-line Survey of Well-being of Young Children (SWYC) about their infants’ motor experience. Nanit uses auto-videosomnography and computer vision algorithms to calculate and report summary statistics of infants’ sleep. Nanit replicated all expected developmental trends in sleep, while the SWYC only replicated some, appearing to underestimate infants at the transition between motor milestones. However, we replicated some well-established findings regarding the relationship between sleep and motor development using both measures, suggesting that both Nanit and the SWYC can feasibly be used for remote data collection, especially if some modifications were made to how researchers explained using the SWYC to caregivers. Researchers should consider alternatives to traditional research methods not only when in-person participation is difficult or impossible, but also when large samples are needed and/or simple measurement tools are sufficient.
