We read with great interest the recent article by Bruss J et al. on hemolysis induced by pulsed-field ablation (PFA) systems. The authors present compelling evidence that hemolysis varies substantially between catheter designs, with focal catheters such as the lattice-tip focal catheter (LTFC) resulting in significantly less hemolysis than large-footprint catheters like the pentaspline (PSC) and variable-loop circular catheter (VLCC). This analysis provides important insights into an under-recognized safety consideration, particularly as PFA continues to gain clinical momentum. We would like to highlight an additional and potentially modifiable contributor to hemolysis: intravenous fluid (IVF) administration.
