IntroductionJuvenile Polyposis Syndrome (JPS) is a rare genetic disorder with a high risk of colorectal cancer, with an incidence rate ranging from 1 in 100,000 to 1 in 160,000. The lifetime risk of developing colorectal cancer in affected individuals can be as high as 39%–68%[1, 2]. Macroscopically, JPS is characterized by polypoid lesions that are pedunculated, exophytic, and smooth-surfaced, while histologically, the polyps are composed of dilated glands rich in mucus and accompanied by inflammatory cell infiltration [3]. According to the NCCN guidelines ”Genetic/Familial High-Risk Assessment: Colorectal,” individuals who meet at least one of the following criteria can be clinically diagnosed with JPS: (1) the presence of ≥5 juvenile polyps in the colon; (2) multiple juvenile polyps in the gastrointestinal tract; (3) at least one juvenile polyp in an individual with a family history of JPS [4]. Patients who meet the clinical diagnostic criteria for JPS typically carry pathogenic mutations in the BMPR1A or SMAD4 genes. SMAD4 is a key intracellular mediator of the transforming growth factor-β (TGF-β) superfamily signaling pathway [5], and the BMPR1A receptor is also a member of the TGF-β superfamily. Studies have shown that approximately 20% of JPS patients harbor pathogenic mutations in the coding regions of the BMPR1A or SMAD4 genes [6]. Currently, most clinical cases of JPS are associated with pathogenic mutations in either the BMPR1A or SMAD4 genes. Here, we report a novel de novo BMPR1A frameshift mutation (c.802_806delinsA; p.E268Tfs*14) in a three-generation JPS family and discuss its clinical implications. .

IntroductionThe urachus is a vestigial embryologic structure resulting from the obliteration of the allantois, which connects the fetal bladder to the umbilicus [1]. Failure of this canal to completely involute after birth leads to a spectrum of urachal anomalies, including patent urachus, sinus, diverticulum, and cyst, with the cystic form being the most common variant [2]. Urachal cysts are typically located in the extraperitoneal space between the bladder dome and the umbilicus and often remain clinically silent throughout childhood [1, 3].The most frequent complication that renders these anomalies symptomatic is secondary infection [4], which can mimic other common causes of acute abdomen, such as appendicitis, Meckel’s diverticulitis, or mesenteric cysts [5]. This non-specific presentation, characterized by suprapubic pain, fever, and a palpable infra-umbilical mass, can lead to significant diagnostic delays [6]. While chronic inflammation may result in adhesion to adjacent structures, progression to a severe inflammatory phlegmon causing bowel ischemia and necessitating intestinal resection is an exceptionally rare and poorly documented complication in the pediatric population.Herein, we report the case of a 14-year-old male with a long-standing infected urachal cyst that precipitated segmental small bowel ischemia requiring en-bloc resection and primary anastomosis. This report aims to highlight this severe and unusual complication, underscoring the potential for significant morbidity and the critical importance of timely diagnosis and definitive surgical management for symptomatic urachal anomalies.

1.IntroductionThe most common causative agents in CNS infections are bacteria, accounting for 36.70%-63.20% of cases[1], with S.aureus being the most common, and fungal brain abscesses being less common in brain abscesses. Fungal brain abscesses are relatively rare in brain abscesses, accounting for 1.05%-2.38% of cases. Candida is one of the four common fungal genera. Candida is common in Candida albicans, and Candida glabrata (C. glabrata) brain abscesses are very rare[2-3]. Due to its extremely low incidence and lack of specific clinical manifestations and imaging features, the diagnosis of C. glabrata brain abscess is very difficult, and the lack of comprehensive and in-depth clinical and imaging knowledge of it makes it easy to misdiagnose it during diagnosis and treatment as a bacterial brain abscess or other diseases or the failure to choose the appropriate antibiotics in treatment. Based on its cytologic features, it can be diagnosed by cerebrospinal fluid pathogenesis. To the best of our knowledge, few articles have reported diffuse multiple critical C. glabrata brain abscesses. Here, we report a case of multiple C. glabrata brain abscess with typical lesion features of C. glabrata brain abscess on cranial Magnetic Resonance Imaging(MRI) and cranial MR contrast enhancement, which was confirmed by lumbar puncture cerebrospinal fluid pathogenesis.
