Introduction:Since the beginning of the COVID-19 pandemic, health professionals have recognized the many ways that COVID-19 can have wide-ranging health impacts. Early in the pandemic, it was recognized that those with diabetes were at higher risk of acute complications associated with SARS-CoV-2 infection.1 However, as the pandemic progressed, a wider range of impacts on glucose metabolism became apparent. Specifically, COVID-19 infections have been associated with new onset Type 1 Diabetes (T1D), Type 2 diabetes mellitus (T2D), worsening glycemic control among those with pre-existing diabetes, and increased incidence of diabetic ketoacidosis (DKA).2,3Given the high prevalence of diabetes and the endemic nature of SARS-CoV-2 in the population, understanding the implications of a COVID-19 diagnosis on the management of diabetes both during an acute infection and in the post-infection period is critical. We present a case of a 58-year-old woman with a 24-year history of well-controlled T2D who presented with sudden, worsening glycemic control following a mild SARS-CoV-2 infection and subsequent glycemic recovery. Written informed consent was obtained from the patient to publish this report in accordance with the journal’s patient consent policy.

AbstractWe report a case of a patient with giant cell arteritis, which was thought to be in remission at the time, who presented with thunderclap headache.   He died eight hours later and was found at autopsy to have a type A aortic dissection. At no point during this short history did he develop chest pain, back pain or breathlessness.  There was no focal neurology. We note that six recent reviews of thunderclap headache do not mention aortic dissection as a possible cause, but have found four case reports of patients like ours whose thunderclap headache was a consequence of aortic dissection in the absence of chest pain. We believe our case, together with those previously reported, supports the view that aortic dissection should be added to the differential diagnosis of thunderclap headache, particularly if the patient also happens to have a history of GCA.

IntroductionEctopic ACTH secretion accounts for approximately 10–20% of cases of ACTH-dependent Cushing’s syndrome and is most commonly associated with pulmonary neuroendocrine tumours and small-cell lung carcinoma (1). Gastrointestinal sources are rare and frequently under-recognised, particularly in cases without classical phenotypic features of hypercortisolism (1,3).Mortality in untreated ectopic ACTH syndrome is largely driven by severe metabolic complications, thromboembolism, and infectious sequelae secondary to profound hypercortisolism (2). Early recognition is therefore essential.In acute and general medical settings, electrolyte disturbances in oncology patients are commonly attributed to gastrointestinal losses, chemotherapy-related effects, or poor oral intake. Failure to recognise hormonally mediated causes may result in repeated admissions, delayed diagnosis, and increased morbidity. This case illustrates how ectopic ACTH secretion may present primarily as severe metabolic instability rather than overt Cushingoid phenotype.
