Following an open globe injury during training, the patient experienced significant visual impairment despite initial surgical interventions. Conventional approaches proved inadequate due to extensive iris loss and intraocular lens instability. We employed a novel dual segmental aniridia prosthesis (SAP) implantation technique, achieving complete functional and aesthetic restoration. Postoperatively, visual acuity improved from 20/40 to 20/25, with reduced glare and enhanced night driving ability. At 6-month follow-up, no complications were observed. This case demonstrates the potential of multiple prostheses in managing complex iris defects, highlighting the importance of adaptive surgical strategies in challenging ocular reconstructions.
