Prosthetic tricuspid valves make permanent pacing challenging. We
describe a case of a patient with several previous heart surgeries with
a mechanical tricuspid valve and a previous malfunctioning left
ventricle lead and recurrent syncope. A quadripolar lead was placed in
the coronary sinus, using a specific left ventricle sensing algorithm.
