This case of a 32 year-old-man with recent radiofrequency ablation presenting with chest pain highlights the diagnostic challenge of atrioesophageal fistulas. Management should include blood cultures, complete blood count, serial computed tomography of the chest with intravenous and oral contrast, broad spectrum antibiotics and referral to a cardiothoracic surgeon.
