Complete surgical removal of a large retrovesical pelvic cyst with an unusual extension could be technically demanding with the high risk of injury to the adjacent vital structures. Opting a conservative approach may reduce morbidity, as the reported incidence of malignancy is very low. This operative technique demonstrates that no evident arising point could be identified on diagnostic laparoscopy, hence the cyst was drained vaginally with partial excision and marsupialization. Histological analysis confirmed a benign epidermoid cyst and there is no recurrence to date.
