Aortic valve regurgitation complicating blunt chest trauma is rare. We describe a case of a 73-year-old male with history of car accident complicated by sternal fracture in 2002. In 2019 he was a candidate for liver metastasis resection complicating adenocarcinoma of the rectum but, for the finding of a holosystolic murmur, he underwent trans-esophageal echocardiography. This showed severe aortic regurgitation due to the fracture of the right coronary cusp. Heart Team decided upon the replacement of the aortic valve with biological prosthesis. Probably, the blunt chest trauma provoked an initial right cusp lesion and the regurgitation worsened over time.
