Brugada patterns are masked and usually are due to provocative factors. We present a case of a 23-year-old Caucasian male patient with a long-standing history of substance abuse who presented with a syncope. EKG performed by EMS showed Brugada EKG pattern.

A patient presenting with worsening dyspnea and left-sided chest pain underwent heart catheterization, found to have a rare connection between the right and left coronary arteries draining into the left ventricle, consistent with dual coronary-cameral fistula.

Asymptomatic cardiac neoplasms are often diagnosed incidentally. Right atrial mass close to septum needs careful evaluation and multimodality imaging to manage appropriately. This case highlights the utilization of multimodality imaging and heart team approach in the management of the right atrial mass.
